Setting sail in the right direction is as easy as 1, 2, 3!

  1. After our service agreement is signed, the client is matched with a care manager whose skill sets best meet the client’s needs.

  2. If necessary, an assessment is conducted at the client’s location and the care manager develops an individualized care plan that includes next step recommendations and referrals, if needed. The client receives a printed copy of this plan.

  3. The client and their dedicated care manager meet to review the plan and decide on the next steps. We can manage some or all aspects of the care plan until the goals are achieved.

Each client or their acting Power of Attorney agent is in charge of approving work to be done. This ensures that everyone is working towards the same goals and there are no breakdowns in communication.
