This is a list of our most requested care management services. You may choose one or more. Additionally, we are happy to discuss client requests for individualized services beyond the basics. Our service goal is to provide care management tailored to our Client’s individual needs.

• Agent under Healthcare Power of Attorney document

• Assist families with oversight of their aging parent

• Assess and develop goal and person-centered care plan to identify next steps needed to resolve pending issues

• Referrals and assistance hiring and managing caregivers

• Identify and recommend ways to combat loneliness

• Companionship via in-person visits and phone calls

• Help find, vet and apply for long-term living arrangements

• Manage relocation and downsizing projects

• Organization and file management

• Inventory of collectibles and household items

• Care coordination going home from rehab and hospital stays

• Attend appointments with Client and manage follow-up recommendations, assure new medications are available and home medication list updated

• Coordinate family support and provide referrals for dementia diagnosis

• Well-being checks at home, hospital, rehab and facility

• Assess current facility and make recommendations to ensure and enhance the standard of care

• Inventory, removal and safe disposal of unused medications to prevent medication mix-ups

• Manage information and communications with doctors via healthcare portal(s)

• Help locate and organize estate planning documents

• Referrals to attorneys in your area who practice specific areas of law (estate planning, Medicaid, SSI/SSDI, special needs, divorce, real estate, etc.)
